Itemization Statement of Funeral Services and Merchandise Selected: The following are the charges for the services, merchandise and livery you selected. You will not be charged for any item, which you have not chosen unless the item becomes necessary due to the other selections you have made. Any such charges are explained below. If we charge for embalming, we will explain why below. (Indicate N/A for items of service and/or merchandise not provided.)

New York State General Price List: effective 11/10/2009 : The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. If legal or other requirements mean you must buy items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral services and merchandise you selected. The list may not include prices for certain items that you select, such as cemetery, crematory charges, flowers or newspaper notice. The estimated final price for those items will be shown on the itemization statement describing the funeral services and merchandise that you selected. We charge you for our service in buying certain items.

I. Funeral Home Charges:

I.A. Alternative Services: (Includes 1 hour Memorial visitation with additional hours charged at the hourly rate for supervision & facilities.)

I.A.1…… Direct Cremation: Price range from $2426. to $2478. ( Long Island+ $1.per mile/per person). The direct cremation prices do not include the crematory charge. If you want to arrange a direct cremation, you may use an unfinished wooden box or an alternative container. Alternative containers can be made of heavy cardboard, pressed wood, or composition materials (with or without an outside covering), or may be pouches of canvas or other materials. Price for a direct cremation where the purchaser provides the container or uses a free used shipping container $2426., price with one hour private visitation (with additional hours charged at the hourly rate for supervision and facilities) $2427., and price for direct cremation using a Cremation/Burial Tray $2478.

I.A.2…… Direct Burial: Price range from $2426. to $2478. ( Long Island+ $1.per mile/per person). The direct burial

prices do not include cemetery charges. Prices for direct burial where the purchaser supplies the casket or uses a free used shipping container $2426., price with one hour private visitation (with additional hours charged at the hourly rate for supervision and facilities) $2427., and for direct burial using a Cremation/Burial Tray $2478.

I.A.3…… Body Donation: ( Stony Brook Hospital – requires their acceptance) no funeral home charges.

I.B…… Transfer of remains to the funeral establishment including personnel, equipment and vehicle: $210. (Long Island +/- $2.50 per mile).

I.C. Preparation of remains:

I.C.1…… Embalming: $899. (In NYS, embalming is not required by law. If you do not want embalming, you have

the right to choose an arrangement which does not require embalming, such as a direct cremation and direct burial. If you select certain funeral arrangements, such as viewing or an open casket funeral, embalming may be required by the funeral firm.
