           Includes the "average" of all of our charges from a local place of death to a local place of cremation &/or burial.  Does Not include non funeral home charges: cemetery, crematory, etc.

           Also Includes: preparation of remains, basic arrangements, supervision & facilities for an afternoon & evening of public visitation, 100 laminated memorial cards, a register book, & a box of acknowledgement cards. 

           For Cremation only, also includes:  an "average" rental casket & a special urn

           For Burial only, also includes:  an "average" purchased casket, a hearse, supervision & facilities for a morning of public visitation, & a graveside service.           

Itemization Statement of Funeral Services and Merchandise Selected: The following are the charges for the services, merchandise and livery you selected.  You will not be charged for any item, which you have not chosen unless the item becomes necessary due to the other selections you have made.  Any such charges are explained below.  If we charge for embalming, we will explain why below.  (Indicate N/A for items of service and/or merchandise not provided.)

New York State General Price List: effective 3/27/2011 : The goods and services shown below are those we can provide to our customers.  You may choose only the items you desire.  If legal or other requirements mean you must buy items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral services and merchandise you selected.  The list may not include prices for certain items that you select, such as cemetery, crematory charges, flowers or newspaper notice.  The estimated final price for those items will be shown on the itemization statement describing the funeral services and merchandise that you selected.  We charge you for our service in buying certain items.

        I.A.1…… Direct Cremation: Price range from $2526. to $5081. (Nassau/Suffolk).  includes the transfer of remains, services of the funeral director & staff, necessary authorizations, the transportation of the remains by livery to the crematory, and the return of the cremains to the funeral home. Direct cremation prices do not include the crematory charge.  If you want to arrange a direct cremation, you may use an unfinished wooden box or an alternative container.  Alternative containers can be made of heavy cardboard, pressed wood, or composition materials (with or without an outside covering), or may be pouches of canvas or other materials.

Price $1996., and price with one hour private visitation (with additional hours charged at the hourly rate for supervision and facilities) $1997., price with embalming $3192. Public transportation (by common carrier) will be charged as a “Cash Disbursement”.

            I.J.2…… Receiving of remains from another funeral home: $250. to $1997. ( Long Island+ $1.per mile/per person).  Price

includes transfer of remains to funeral establishment and/or place of local disposition, securing necessary authorizations, the service of the funeral director and staff.  Receiving of cremated remains: $250, or with one hour private visitation for cremated remains: $251. Receiving remains price $1996., and price with one hour private visitation $1997 (with additional hours charged at the hourly rate for supervision and facilities). Public transportation (by common carrier) will be charged as a “Cash Disbursement”.
